Purpose
The Total Bilirubin Test is a blood test that measures the overall level of bilirubin in the blood, including both conjugated (direct) and unconjugated (indirect) bilirubin. The body produces bilirubin as it breaks down red blood cells, and the liver processes and eliminates it.
This test is important for evaluating liver function, diagnosing jaundice, and identifying liver diseases such as hepatitis or bile duct obstruction. It also helps monitor liver health in patients with known liver disorders.
The test can be combined with Direct and indirect bilirubin levels, Liver function tests, Complete blood count (CBC), Hepatitis panel, and Prothrombin time (PT).
Patient Preparation
The patient is required to avoid eating or drinking anything other than water four hours before the test.
Samples
1ml with a minimum volume of 0.7ml of Serum or plasma collected in a Gel-barrier, red-top, or green-top tube (lithium heparin). Separate the plasma or serum within 45 minutes of sample collection. Room temperature storage is only recommended for up to 3 days.

Reference Range
24 hours <8mg/dL, 48 hours <13.2, 72 hours<15.6mg/dL, 96hours -1 month <16.6 mg/dL ,>I month and adults <1.2mg/dL.
Results Turnaround
Within 24 hours of receipt of samples.
Diseases / Diagnostic indicators
Elevated total bilirubin levels
May indicate conditions that affect the liver, bile ducts, or red blood cells. These include:
- Liver diseases such as hepatitis, cirrhosis, or liver damage due to alcohol or toxins
- Bile duct obstruction, as seen in gallstones or tumors
- Hemolytic anemia, where rapid red blood cell breakdown increases bilirubin levels
- Neonatal jaundice, common in newborns due to immature liver function
Lower-than-normal bilirubin levels are rare and typically not clinically significant, but may occasionally be seen in certain medications or as part of a broader liver function pattern.
High total bilirubin:
- Suggests impaired liver function, bile flow obstruction, or increased red blood cell breakdown.
Normal bilirubin:
- Indicates healthy liver function and normal red blood cell turnover.
Mild elevation:
- It may be temporary or related to Gilbert’s syndrome (a benign liver condition).
